I am a case manager and professional trainer with over 15 years of experience working with community members and partner agencies to improve health and social outcomes. I create client-centered case plans and coordinate services including housing, vocational support, nutritional assistance, and referrals.

I have a strong track record in outreach and care management, completing thousands of health follow-ups and home visits while building trusting relationships with patients and caregivers. I proactively encourage members to manage their health and collaborate closely with primary physicians and community partners.

As a trainer and preceptor, I developed month-to-month training plans, facilitated weekly meetings, and trained 50–60 healthcare ambassadors, supporting new hires with benchmarking and skill development. I also design and deliver workshops, training materials, and informational resources for both staff and clients.

I am proficient in Microsoft Office, Rosalind database and EMR-related tools, CPR/First Aid certified, and experienced in HEDIS, ICD-10 entry, med reconciliation, and referral coordination. I bring dependable, organized, and resourceful leadership to teams focused on community-based care.
