Provide ongoing companionship and support to an elderly resident in a nursing home, assist with coordinating basic needs, and advocate for resident well-being and emotional support.

Provide spiritual mentorship to USC students, facilitate small groups, lead outreach efforts, and create spaces for fellowship and personal development. Maintain documentation, collaborate with community partners, and ensure individuals receive appropriate support services.

Served as primary contact for California Children's Services, acute care hospital discharge planners, and community partners under the Whole Child Model Program. Completed assessments and routed high-risk members to complex case managers.

Provided health education to youths with chronic conditions and mental/behavioral health needs. Conducted health and wellness assessments, addressed barriers, created goals, and linked prevention, primary care, and mental health services.

Conducted outreach to low-income women of child-bearing age, provided referrals and community resources, conducted public health workshops, completed health screening forms, and promoted the county's first-time mom home visitation program.
