I've supported patients with chronic conditions, including HIV/AIDS, at The Alliance for Positive Change since 2014. I complete intakes and eligibility verifications, build individualized care plans, and coordinate appointments, referrals, and discharge planning.

I navigate Medicaid, Medicare, and commercial insurance barriers to help patients access medications and treatment. I also advocate in housing court and through social service and insurance appeals to resolve benefit denials.

I've managed caseloads of 50+ clients while maintaining accurate Health Home records and supporting treatment adherence through home visits, virtual visits, phone check-ins, and follow-ups. As an agency trainer, I bring experience in patient advocacy, harm reduction education, and community-resource navigation.

Earlier, I taught medical billing and coding, handled insurance verification and claims at a VA Hospital, and provided home care support for homebound patients.
