At Accordant Health, a CVS Health company, I've provided remote telephonic case management and health management since 2005 for members with complex, chronic, and rare conditions. I assess clinical needs, build member-centered care plans, coordinate providers and specialty pharmacies, and help members access prescribed therapies.
Across 28 years in nursing, I've also delivered home health and hospice care, coordinated MDS/RAI care plans, and supported outpatient surgical patients. As an RN Preceptor and Epic Superuser, I've trained nurses, contributed to technology implementation and testing, and consistently met quality, productivity, and efficiency expectations.
