I’m looking for a role where I can coordinate community partnerships, manage high-impact program operations, and ensure compliance and reporting—while leading teams and improving access to healthy food and essential resources.

I’m a Case Manager & Outreach Specialist focused on improving access to essential resources through strong community partnerships and coordinated program operations. In my current role, I coordinate services across healthcare providers, nonprofits, housing nonprofits, and community partners, while maintaining accurate documentation, reporting, and compliance to support efficient service delivery.

Previously, I served as a Community Health Manager for a Medicaid-funded food access program serving 160 participants, building relationships with healthcare providers, local farms, nonprofits, and community organizations. I supervised seven staff members and coordinated more than 30 volunteers, improved reporting systems and operational workflows, and also brought a business-operator mindset from running a regenerative farm—managing budgeting, inventory, CSA memberships, scheduling, and order fulfillment.
