At LIFEPlan CCO NY, I manage complex care for approximately 40–41 members with medical, behavioral health, developmental disability, and community-based support needs. I develop and monitor more than 40 individualized Life Plans while serving as a primary contact for members and families.

I coordinate physicians, specialists, hospitals, therapists, residential providers, schools, community organizations, and government agencies. I also manage 50+ weekly communications and support appointments, referrals, transportation, hospital discharges, and transitions of care.

I help members and families navigate Medicaid, Medicare, SNAP, SSI, recertification, and public-benefit needs, including coordination with the Rockland County Department of Social Services. Using Motivational Interviewing and person-centered communication, I support engagement, self-advocacy, informed choice, and individualized goals.

My healthcare background also includes mobile patient services at Drip Hydration, community health outreach at New Destiny Family Success Center, and residential supervision at Community Access Unlimited. I maintain HIPAA-compliant documentation in CX360 and eVero and mentor newly hired Care Managers on workflows, engagement, healthcare navigation, and community resources.
