At Community Healthcare Network, I manage a caseload of 55 patients and provide ongoing care management in line with program standards. I conduct comprehensive assessments and develop patient-centered care plans with input and approval from patients and their healthcare providers.
I collaborate with patients and interdisciplinary care teams to coordinate services and support progress toward established goals. I also conduct home and field visits to assess needs and connect patients with appropriate resources.
At Samaritan Daytop Village, I met with clients to develop and review Independent Living Plans and connected them with healthcare, mental health, and community resources. Earlier, I planned youth activities at Samaritan Daytop Village and created leadership and STEM lesson plans at Aspira NYC.
