What I'm looking for

I'm looking to continue coordinating equitable, patient-centered care for medically complex and underserved populations, using whole-person care planning, Medicaid navigation, interdisciplinary collaboration, and community resource connections to close care gaps.

At Community Health Network of NYC, I manage 70+ NYS Health Home patients with complex chronic conditions, coordinating medical, behavioral, and social support while closing 40+ care gaps each quarter.

I've also supported 150+ Medicaid managed care patients at BronxCare Health System and high-volume patient services at Essen Health Care. I bring hands-on experience in care plans, transitions, benefits navigation, community resources, EHR compliance, and patient-centered outreach across diverse New York City communities.
