I’m looking for a role where I can use my experience in healthcare, case management, and client support to make a meaningful impact. I value collaboration, empathy, and continuous learning, and I’m especially interested in remote opportunities that support professional growth and exceptional client care.

I’m a healthcare and human services professional with experience spanning case management, patient navigation, care coordination, public benefits, and program administration. Throughout my career, I’ve helped individuals navigate complex healthcare and social service systems by connecting them with resources, coordinating care, and advocating for positive outcomes.

My background includes targeted case management, Medicaid eligibility determination, patient navigation, public assistance programs, housing services, and education-based programming. I’ve collaborated with physicians, behavioral health providers, social workers, community organizations, and multidisciplinary teams while supporting diverse populations with medical, behavioral, and social needs.

I have extensive experience managing documentation, developing care plans, coordinating referrals, maintaining compliance, conducting outreach, and building trusting relationships with clients. I’m proficient in Microsoft Office, Salesforce, NextGen, Phreesia, OnBase, CoverMyMeds, WMS, ePACES, and other healthcare and case management platforms.
