Manage a 90-120 member statewide caseload, providing trauma-informed bilingual support across medical, behavioral, social, and insurance barriers. Led preventive-health gap-closure initiatives to the #1 Q4 2025 performance ranking and maintain a 40%+ needs-met rate with 90+ engaged members monthly.

Helped launch the Case Management Pilot Program and enrolled/supported 350+ asylum seekers across CMPP and the Migrant Relocation Program. Conducted intakes, service planning, mental-health and human-trafficking screenings, referrals, and ongoing case management.

Provided bilingual healthcare navigation across six sites and enrolled 150+ consumers per month through the NY State of Health Marketplace. Resolved insurance and enrollment needs using NYSOH, ePaces, and eClinicalWorks while supporting individuals, families, and small businesses.

Managed remote learning-center operations, staff training, community culture, and monitoring of students' academic progress. Developed youth arts/wellness programming and summer events in collaboration with the Upper Nyack Department of Education.
