At VRD Group, I coordinate community-based services for Medicaid waiver participants, addressing care needs, service gaps, and barriers to support. I develop and monitor Required Service Plans, Individual Service Reports, Plans of Protective Oversight, and related person-centered documentation.
I also conduct member and caregiver outreach and organize interdisciplinary team meetings with home care agencies, clinicians, and other providers. I track UAS/LOC documentation, authorizations, eligibility, and renewals to support continuity of services.
At Center for Independence of the Disabled, New York (CIDNY), I coordinated transitions from institutional settings to community living. I worked with members, families, agencies, managed care plans, housing partners, and providers to address transition barriers and connect people with ongoing supports.
My earlier work included healthcare case management operations, Medicaid program quality assurance, and healthcare program coordination. I hold an MBA in Healthcare Leadership & Management and a BS in Human Services from SUNY Empire State University.
