I’m seeking a remote Nurse Case Manager/Care Manager role focused on utilization review, transitional care, and care coordination—partnering with interdisciplinary teams to reduce barriers, manage complex caseloads, and improve outcomes.

I’m a Registered Nurse Case Manager with 6+ years of experience in acute care, emergency nursing, inpatient case management, utilization management, discharge planning, care coordination, occupational health, and transitional care. I collaborate closely with interdisciplinary teams, physicians, payers, patients, and families to coordinate safe, cost-effective care across the continuum.

I manage complex caseloads, conduct utilization reviews, facilitate care transitions, and reduce barriers to discharge to improve patient outcomes and length-of-stay metrics.
