What I'm looking for

I’m looking for case management and medical review work where I can use telephonic assessment, strong documentation, and care coordination to support safe transitions of care, remove barriers, and improve compliance and outcomes.

I’m a Registered Nurse with over 3 years of experience across hospital acute care, post-acute settings, and remote care management. I bring a strong clinical foundation from progressive care and step-down environments into telephonic health and care management.

In my current Registered Nurse Care Manager role with Sagility, I provide telephonic care management for Special Needs Plan (SNP) members with complex medical and psychosocial needs. I perform ongoing patient assessments and post-discharge evaluations, including review of discharge instructions and medication reconciliation, and educate members on red flag symptoms to support safe transitions of care.

I also strengthen outcomes by addressing social determinants of health and coordinating resources to remove barriers to care. Across settings, I collaborate with interdisciplinary teams and use chart review and documentation audits to ensure clinical accuracy, compliance, and continuity of care.

I’m especially well positioned for case management, utilization review, and medical review roles. I’m known for clinical judgment, attention to detail, and the ability to communicate complex medical information clearly—while staying organized and focused on patient advocacy.
