At CareFirst BlueCross BlueShield, I direct and coordinate a team of eight remote care managers serving Maryland Medicare DSNP members. I develop care management programs and work with an interdisciplinary team to support quality care and manage health care costs.
As a Case Manager, I engage members and caregivers by phone to implement care plans, support self-management of chronic conditions, and help reduce preventable hospitalizations. I also monitor outcomes, collaborate with providers, and serve as a preceptor and resource for new case managers.
At Penrose St Francis Medical Center, I worked with physical therapy to eliminate CPMs from hospital practice, resulting in a 100% increase in documented post-op Day 0 mobility and patient-performed exercises. I also adjusted staffing grids during a period of 16% volume growth and was involved in implementing an Accelerated Joint Recovery program.
Earlier, at Centennial Medical Center, I designed and implemented an ED-specific orientation manual and assisted with a Low Acuity Area that reduced door-to-doctor times by 77%. Iām a Certified Emergency Nurse with experience in inpatient and outpatient care, clinical operations, and nurse management.
