What I'm looking for

I’m looking for a RN case management role where I can coordinate complex patients, run accurate utilization reviews and care plans, and collaborate with interdisciplinary teams to drive safe discharges and strong post-discharge follow-up.

I’m a dedicated, patient-focused registered nurse with proven expertise in acute patient care, staff development, and family advocacy. I’m known for multitasking through competing priorities while maintaining superior accuracy in patient history, charting, and documentation.

As a case manager at UCLA medical center (2021 to present), I work weekends performing discharges to home, SNF, ARU, LTAC, lateral acute transfer, and home health services. I also support utilization reviews and case management as needed.

Previously, I served as a remote complex case manager for commercial health plan members (Health Net, 2017–2022). I conduct health risk assessment, diagnosis and medication reconciliation, create care plans, and complete post-hospital discharge assessments—then coordinate disease management and referrals (palliative, dietitian, LCSW, and mental health therapist) as needed.

I bring long-standing telephonic and acute-care case management experience across Anthem Blue Cross, City of Hope, and other managed healthcare organizations. I hold ACM certification in case management and bring advanced proficiency with MCG and Interqual guidelines, plus strong documentation and utilization tools including Epic, Careconnect, and Meditech.
