I’m a Registered Nurse experienced in case management, care coordination, and clinical patient assessment across home health, ICU, and long-term care settings. I manage high-risk, complex populations by developing and implementing individualized care plans that support continuity of care.
In my RN Case Manager role, I perform comprehensive assessments, develop tailored care plans, and conduct ongoing case management and patient monitoring. I coordinate transitions of care between providers, families, and healthcare services while delivering telephonic and in-home support, education, and follow-up.
I’ve also supported complex discharge planning and transitions of care in a high-acuity CVICU environment. There, I applied the nursing process for assessment, planning, implementation, and evaluation of care, working closely with interdisciplinary teams to ensure appropriate resource utilization and compliance.
Earlier in my career as an LPN, I delivered long-term care and chronic disease management for a geriatric population, including monitoring and documentation of patient conditions such as wounds and skin integrity. I supervised CNAs to maintain continuity of care and patient safety—building a strong foundation for the collaborative, detail-driven work I do today.
