What I'm looking for

I’m seeking a remote role where I can coordinate chronic care end-to-end—patient outreach, RPM tracking, and HIPAA-ready EHR documentation—while supporting multidisciplinary teams and improving patient satisfaction through consistent, audit-ready follow-through.

I’m a licensed pharmacist (B.Pharm) and Certified Medical Virtual Assistant with 2+ years of remote clinical care coordination experience. I support chronic care management by monitoring patient progress and documenting monthly chronic care outcomes for multidisciplinary teams.

In high-volume work, I complete 10–12 patient calls per hour for assessments, follow-ups, and care plan education. I update patient medical histories and maintain audit-ready documentation across EHR/EMR systems, including Athenahealth, DrChrono, and eClinicalWorks.
