What I'm looking for

I’m looking for a community-health or care-coordination role where I can combine patient navigation, Medicare/Medicaid enrollment support, and trauma-informed communication to improve access, documentation, and outcomes for vulnerable families.

I’m a community health professional and licensed health insurance agent with extensive experience in patient navigation, case management support, and Medicare/Medicaid enrollment. I’m known for strong client advocacy, crisis support, and care coordination for vulnerable populations, including pregnant and postpartum individuals, children, refugees, and underserved families.

In remote insurance roles, I provide client-centered Medicare consultation and plan education, conduct high-volume inbound and outbound calls, and explain coverage and eligibility in compliance with CMS regulations. I maintain accurate documentation in CRM systems and support regulatory adherence through HIPAA-compliant, confidential records.

As a Patient & Community Health Navigator with Brooklyn Perinatal Network, I helped patients with insurance enrollment, renewal, and healthcare access navigation across Medicaid and Marketplace plans. I resolved coverage issues and connected pregnant and postpartum clients to maternal health programs, doulas, housing, and social services while coordinating community outreach events and follow-ups for continuity of care.

I’m currently pursuing a Master of Social Work (MSW) to expand my clinical and behavioral health practice. I also bring program leadership experience—supervising MSW student interns and coordinating workshops, outreach materials, intake interviews, and referrals to support measurable program participation and service delivery improvement.
