At Pyx Health, I conduct bilingual outreach to understand members’ healthcare and social service needs, then connect them with providers, benefits programs, and community resources. I developed 240 individualized care plans while maintaining documentation and follow-through despite many members being unreachable.
As a Peer Mentor at Pyx Health, I managed a remote caseload of more than 100 members living with diabetes and other chronic health conditions. I provided culturally responsive health education and coordinated referrals to professionals, support groups, and community services.
At ADVANCE | Lake Tahoe Community College, I provide bilingual transition navigation and case management to adult learners pursuing education, training, and employment goals. I assess individual needs and develop pathway plans around each learner’s goals, strengths, and barriers.
Earlier, I supported callers facing mental health challenges at the National Alliance on Mental Illness (NAMI), using motivational techniques and de-escalation, and connecting people at risk with crisis and emergency services. My experience also includes workforce development at JOIN, Inc. and trauma-informed client services at Live Violence Free.
