What I'm looking for

I am looking for remote or hybrid work with a focus in healthcare, including case management, policy, and program management.

At Lifemark Health Group, I provide case management for veterans with complex medical and psychological needs to empower them in rehabilitation towards return to work and return to community goals. I analyze medical documentation to determine rehabilitation needs and goals, then coordinate services to achieve these goals. I also review program data and outcomes to help improve operations and effectiveness.

At Pleasance Physiotherapy, I assessed patient function, developed and implemented treatment plans, and analyzed outcomes for adult and geriatric patients. I also built community partnerships to support referral pathways and collaborative care.

At Impact Healthcare, I provided pelvic health and orthopaedic physiotherapy, including care for MVA and WSIB clients. I also led community outreach seminars and staff presentations.
