What I'm looking for

I’m looking for a role where I can combine clinical support with chronic care management—coaching patients, coordinating providers, maintaining strong EMR documentation, and supporting smooth transitions of care in both remote and in-person settings.

I’m a Registered Medical Assistant with extensive experience in care coordination, patient education, chronic care management, and clinical support. I build personalized care plans, set realistic goals with patients, and keep documentation accurate in electronic health records.

In my recent role, I coordinated care between patients, providers, specialists, and ancillary services to ensure continuity of care. I also supported transitions of care from hospital to home to reduce readmissions, conducted follow-ups to monitor progress, and connected patients to community resources addressing social determinants of health.

Across my medical assistant work, I’ve evaluated patient needs, managed patient flow, collected specimens, performed lab testing, and prepared for non-invasive minor procedures. I’ve also administered medications and injections, performed EKGs and obstetric testing (including fetal heart tones via Doppler and non-stress tests), and worked with multiple EMR systems such as Docutap, Digi, IMS, Intergy, and Athenahealth.
