What I'm looking for

I’m looking for a role that blends clinical nursing with care coordination—especially pediatric-focused case management—where I can advocate for patients, uphold patient safety, and collaborate with teams to improve quality and outcomes.

I am a Registered Nurse with experience in pediatrics and with adults, combining patient-centered clinical care with case management. In my current role as a Pediatric Nurse Case Manager, I assess members’ needs through in-home physical, psycho-social, and environmental evaluations and identify barriers to care and social determinants of health.

I coordinate and facilitate care to support members in achieving individualized short- and long-term goals. I advocate within the care coordination program, schedule follow-up calls/visits, make appropriate referrals, and build plans of care that use quality and cost-effective resources.

Previously, I supported pediatric patients through contract nursing and delivered direct bedside care by measuring vital signs, performing assessments, documenting outcomes, and providing patient/family education. I also served as a Clinical Supervisor, monitoring clinic quality measures (UDS, HEDIS, PCMH) and supporting staff recruiting, training, scheduling, and coaching to maintain consistent standards and compliance.
