Conducts comprehensive clinical, functional, and risk assessments for Medicare and Medicaid members to develop individualized plans of care. Manages a caseload of 50-60 members, coordinates services, and performs transitional care follow-up to reduce readmissions.

Supervised daily clinical operations for 20+ complex home care patients and nursing staff, ensuring quality, safety, and regulatory compliance. Reviewed physician orders, care plans, and nursing documentation, and provided clinical guidance and education to staff.

Delivered skilled nursing care to medically complex pediatric and geriatric patients in the home setting. Performed comprehensive patient assessments, educated patients and caregivers, and coordinated care with physicians and community resources.

Supported primary care services for pediatric patients through assessment, patient education, immunization administration, and follow-up care. Assisted with triage, appointment coordination, and provider support in a fast-paced outpatient setting.

Currently pursuing a Master of Science in Nursing with a focus on Public & Population Health.
