Hired as part of a Medicaid expansion program for Geisinger Health Plan. Case managed up to 400 patients as the program was just being built. Assess patients as they are discharged from a hospital/rehab/SNF and prevent hospitalizations.

Returned to the field nursing and resumed my position as a Case Manager as previously described. Managed a caseload of patients, coordinating care and preventing hospitalizations.

Joined the education team to help promote growth, educate current employees, and assist on the onboarding process of new hires. In charge of education for the Mom/Baby department, Private Duty, and all nursing contractors.

Case managed up to 50+ adult clients providing intermittent visits in the home. Thorough assessment of all systems of each client and review of multiple comorbidities. Excellent communication with physicians, clients, families, and office staff.

Case managed 22 of the 33 active Continuous Care Pediatric clients. Field supervision of all nurses working on these cases along with performing their annual evaluations. Obtained primary authorizations per calendar year/plan year on 22 of the 33 Continuous Care Pediatric patients.

Cared for sick and/or premature infants in a Level III Surgical NICU. Gained experience with ventilator management, intravenous therapy, and blood draws. Assisted Neonatologists and Neonatal Nurse Practitioners with various procedures including Lumbar Punctures and Umbilical Line insertion.

Taught Pediatric clinical to second year students. Monitored students as they assessed the pediatric population, administered medication, obtained vital signs, and interacted with both the patient and their family, as well as nursing staff.

Experienced in being in charge of a 72 bed Level III NICU with regards to staffing and ensuring the safety of both the infants and staff. Assisted in L&D/OR with premature deliveries or signs of distress in any delivery. Assessed and admitted infants.

Assisted in the labor and delivery of infants. Cared for both infants and mothers after the delivery processes. Gained experience in triage and OR, IV starts, assessing infants in distress, and stabilizing infants for transport when needed.

In charge of the Alzheimer's Unit, Assisted Living, and Certified Nursing Assistants. Responsibilities included chart review, triaging calls from independent living residents in their cottages and apartments and acting upon them when necessary. Distributed all medications and performed all treatments.

In charge of the Transitional Care Unit with long Term care patients, critical diabetics, peg tubes, amputations, Alzheimer’s patients. Responsible for filling out the Minimum Data Set (MDS) clinical assessment of all residents when appropriate.

Completed a Bachelor of Science in Nursing, focusing on fundamental nursing principles and patient care. Gained essential skills for professional nursing practice.

Pursued a Master's of Science in Nursing with a concentration in Community Health Nursing and Education. Developed advanced skills in community health interventions and educational strategies for healthcare settings.
