I’m looking for a HIPAA-compliant Digital Health or Telehealth-focused role where I can own care coordination, care navigation, and patient support—partnering with clinical teams to remove barriers, improve outcomes, and strengthen the Patient Experience through process improvement.

I’m a healthcare coordination professional with 6+ years of experience supporting patients and strengthening clinical operations. I focus on HIPAA Compliance, Care Navigation, and Digital Communication to improve outcomes in complex health environments—especially in Digital Health and Telehealth Operations.

In my current role as an Intensive Case Manager, I coordinate Care Navigation for clients in infectious disease sectors, connecting people to Telehealth Operations and community resources. I manage an active caseload of 40+ clients using CRM Software and Data Tracking to monitor progress, maintain continuity of care, and keep documentation accurate, timely, and confidential.
