About the Opportunity
Health Care Social Workers provide a diverse range of interventions to clients and their families, enabling them to achieve their optimal level of functioning and health. These interventions include: psychosocial assessments, adjustment therapy, supportive therapy, crisis counselling, resource counselling, and discharge planning and case management.
The Social Worker will be a part of the INSPIRED COPD outreach program which requires knowledge of issues related to smoking cessation, addictions, grief and loss, anxiety and depression related to chronic disease, end of life, disability, social determinants of health, equity, motivational interviewing, caregiving and continuum of care and intervention skills that come from knowledge of theoretical frameworks/approaches to practice. The MSW must be able to work collaboratively with the team Advance Care Planning Facilitator to coordinate an appropriate treatment/management plan for mutual patients.
About You
We would love to hear from you if you have the following:
- Master's Degree in Social Work from a recognized university
- Licensed or eligible for membership with the Nova Scotia College of Social Workers (NSCSW)
- (2) years of clinical experience as a clinical Social Worker required
- Strong counselling and communication skills
- Knowledge and experience in Smoking Cessation and Motivational Interviewing
- Knowledge of program planning
- Ability to work with a high degree of autonomy
- Ability to work effectively as part of an interdisciplinary team
- Competencies in other languages an asset, French preferred

Hours
- Full-time long assignment, 100% FTE, 75 hours biweekly
- Approximately for (12) months. Dates are subject to change
- This is a hybrid position. Working from home & also working in the community. Option to work from the Bayer's Lake facility office included
Compensation and Incentives
$37.31 - $49.05 Hourly
