I’m a compassionate, results-driven healthcare professional with 10+ years in clinical coordination, maternal health, and community-based case management. I support Medicaid and underserved populations through virtual and in-person care models.

In my current role as a Clinical Coordinator, I coordinate care for patients in virtual, value-based care programs. I serve as a primary point of contact for patient onboarding, education, and ongoing engagement, working with interdisciplinary clinical teams to support individualized care plans and timely follow-up.

Previously, as a Community Health Worker II / Case Manager, I delivered comprehensive Medicaid case management after hospital discharge. I conducted home visits, phone outreach, and follow-ups to re-establish continuity of care, and I addressed social drivers of health by connecting patients to housing, food access, childcare, employment, transportation, and mental health resources.

Earlier, I worked as a Community Birth Worker / Case Manager, providing culturally responsive prenatal education, labor support, and postpartum care while maintaining detailed case documentation and coordinating follow-up. I also coordinated appointments and insurance navigation as a Care Coordinator at VA Women’s Center, reinforcing my strong patient advocacy, relationship-building, and documentation skills.
