What I'm looking for
I’m an experienced Certified Community Health Worker and Suicide Prevention Case Manager with a strong background in clinical and behavioral health settings. In my current role, I manage a caseload of 40+ veterans across low to high suicide risk, using evidence-based biopsychosocial and suicide risk assessments (including C-SSRS) to develop individualized safety plans and service goals. I collaborate closely with VA Suicide Prevention Coordinators, psychiatrists, psychologists, social workers, and multidisciplinary mental health teams to coordinate comprehensive care and provide crisis intervention and prevention counseling.
Before this, I provided community health support across Internal Medicine, Infectious Diseases, Neurology, and Cardiology clinics by connecting patients to resources that address social determinants of health, using Unite Us for referrals. I documented care in Epic and Cerner, managed data with REDCap, and supported patient education for chronic disease prevention, medication adherence, and self-management through culturally responsive communication and motivational interviewing. Earlier experience includes high-volume internal support and direct support for community-based clients, reinforcing my focus on continuity of care, accurate documentation, and patient-centered outcomes.
