I'm a licensed social worker supporting residents in long-term care through admissions, advocacy, care planning, and transition coordination.
At Volunteer of America’s The Homestead at Anoka, I advocate for residents’ rights, preferences, and psychosocial well-being. I coordinate admissions, complete assessments and documentation, and help determine appropriate care levels and placements.
At The Estates at Chateau, I managed the full admissions process, including intakes, assessments, and transitions into the facility. I also developed and maintained individualized, compliant care plans aligned with resident needs.
My experience also includes social services at Bethel Care Center, pre-release support for incarcerated individuals with the Minnesota Department of Corrections, direct resident care as a nursing assistant, and a psychiatric hospital internship.
