Samantha Nosenchuk

I’m a Licensed Master Social Worker with hands-on experience across inpatient behavioral health, outpatient therapy, and geriatric rehabilitation settings. I focus on psychosocial assessment, individualized planning, and clear, timely documentation that supports safe outcomes across the continuum of care.

In my current role as a Licensed Master Social Worker–Temporary Case Coordinator, I conduct psychosocial assessments and develop individualized treatment and discharge plans. I coordinate care with interdisciplinary teams, families, community providers, and insurance companies to ensure effective treatment, resource use, and safe discharge planning.

Previously, I facilitated individual therapy sessions using evidence-based approaches such as CBT and DBT, and I conducted comprehensive member assessments to create individualized care plans. I also provided emotional support and crisis intervention to residents and families, while coordinating referrals to home health, hospice, transportation, and specialized care services.

I bring a strong advocacy and engagement mindset from serving as a Masters in Social Work Student Ambassador, where I represented the MSW program at recruitment events and collaborated with faculty and leadership to support student success. I’m committed to quality improvement, regulatory compliance, cultural competence, and building partnerships that improve retention and follow-up compliance.
