At Logan Square Rehabilitation and Health Care Center, I direct social services operations and coordinate resident-centered care in a skilled nursing and long-term care facility. I develop individualized care plans with residents, families, and the interdisciplinary team.
I coordinate discharge planning from admission through discharge, connecting residents with referrals, home health services, medical equipment, transportation, and follow-up resources. I also provide crisis intervention and emotional support to residents and families.
At Action Wellness, I provided case management for perinatal clients, coordinated healthcare services, and connected people with community resources and public assistance. My experience also includes medical social work, rehabilitation, long-term services coordination, and early childhood education.
