What I'm looking for

I’m looking for a role where I can lead with empathy at first contact, conduct intake and psychosocial assessments, coordinate care with interdisciplinary teams, and maintain compliant documentation—supporting clients through meaningful life transitions.

I’m a compassionate psychology and social services professional with hands-on experience in intake assessment, case coordination, behavioral health support, and client communication. I focus on being a steady, first-point-of-contact presence for individuals and families going through sensitive, emotionally complex transitions.

In skilled nursing and long-term care, I conduct intake and psychosocial assessments, guide residents and families through care transitions, and support discharge planning with referrals to home health, durable medical equipment, and community resources. I maintain accurate documentation in PointClickCare (PCC) in compliance with HIPAA and CMS regulations, while partnering with interdisciplinary teams to develop and update individualized care plans.

As an RBT, I serve as a key point of contact for families—providing education, therapy updates, support, and follow-up communication. I balance fast-paced priorities while maintaining confidential, accurate data entry and treatment records in CentralReach (CRM), and I collaborate with clinical supervisors to support individualized treatment plans.

I also bring earlier field experience from a Social Services Internship and school and administrative roles where I supported vulnerable individuals, handled sensitive conversations professionally, and strengthened my intake-and-referral skills.
