What I'm looking for
I’m a social worker who builds trust quickly and supports people through complex life challenges—especially abuse, neglect, mental illness, and exploitation. I bring strong communication and an advocacy mindset, and I’m motivated to use innovative, community-centered approaches to help individuals move toward stability.
In case management, I conduct family meetings to align goals of care and support high-risk children and families with mental health challenges. I also complete initial and psycho-social assessments, manage intakes, and connect clients to the right mental health and support services to strengthen long-term outcomes and lifestyle management.
In mental health direct care roles, I help plan and evaluate patient needs based on behavior and I provide services in line with treatment plans under nursing direction. I document client progress in confidential files, observe and monitor responses to treatment, and facilitate supportive groups that reinforce positive problem-solving skills in a safe, therapeutic environment.
Across settings—including hospitals, community health clinics, long-term and residential care environments, and hospice-style support—I coordinate care and help clients take practical steps toward self-sufficiency. I’m also certified in crisis prevention and CPR, and I bring experience with medication technician responsibilities to ensure safety, structure, and responsiveness during critical moments.
