As a dedicated and compassionate Care Manager, I have a strong passion for delivering high-quality patient-centered care to vulnerable populations. With a varied background in healthcare, I bring a unique set of skills and experiences to the table. My ability to provide trauma-informed care and support has been refined through my work in primary care settings and with chronically ill patients.

Throughout my career, I have developed a strong foundation in care coordination, referrals, and linkages, as well as education and support. My experience working with patients living with HIV has given me a deep understanding of the importance of empathy and compassion in healthcare. I am committed to providing individualized care that addresses the unique needs of each patient.

In addition to my clinical skills, I am proficient in EMR charting and have experience with COVID-19 screening. I am a strong communicator and team player, with a proven ability to work effectively with interdisciplinary teams to achieve optimal patient outcomes.
