I’m a compassionate, organized, and client-focused case manager with 8+ years of wholistic case management experience. I advocate for patient health and well-being by working closely with healthcare workers, interdisciplinary teams, and institutions.

In my current role as a Care Navigator at Centene Corporation, I evaluate members’ needs and barriers to care, facilitate personalized care plans, and coordinate providers, specialists, and community resources. I also provide psychosocial and resource support while monitoring progress and collaborating on plan revisions.

Previously, as a Support Planner/Case Manager at Medical Management and Rehabilitation Services, I assessed clients for Medicaid covered Home and Community Base Services and Medicaid Waivers, conducted home visits, and intervened as an advocate in crisis situations. I built authorizations for personal care services and ensured accurate documentation in electronic databases.

Earlier, as a Social Work Assistant at Encore at Turf Valley, I managed caseloads across long-term care and acute rehabilitation, coordinated care plan meetings, and supported safe discharges with home health and durable medical equipment. I bring empathy, attention to detail, and strong documentation to every stage of care.
