What I'm looking for

I’m looking for a role where my nurse case management expertise supports smooth, collaborative transitions for complex patients, with strong team communication, professional standards, and opportunities to grow as I complete my Family Nurse Practitioner training.

I’m a board-certified nurse case manager (CMGT-BC) and experienced RN with a strong track record supporting complex transitions—especially for active-duty service members preparing for VA disability claims. In my current role, I help ensure medical concerns are documented and addressed well ahead of separation so claims can be filed efficiently and accurately.

I begin the transition process at least 24 months prior to separation, prepare the required forms and medical addendums, and coordinate documentation with the CMO to close gaps and meet timelines. I connect service members with the appropriate Veterans Service Officer once medical needs are ready, then follow up every three months through one year post–active duty to support continuity of care through local VA services.

Before case management, I built deep clinical expertise across high-acuity settings. I’ve worked in ICU/SDU environments and specialized cardiac and advanced heart failure programs, with experience supporting therapies such as LVADs, total artificial heart care contexts, pulmonary hypertension titration, CRRT, inotropic/vasopressor support, and ventilator management.

I’ve also held leadership and cross-disciplinary care roles, including Clinical RN Manager for an Interdisciplinary Pain Medicine Clinic. There, I managed day-to-day clinic operations, optimized provider schedules, coordinated referrals for active-duty personnel on limited duty and pending medical boards, and participated in ECHO interdisciplinary meetings to support chronic pain weaning strategies.
