Assessed and documented Social Determinants of Health (SDOH) and provided oncology-specific patient education, emotional support, and navigation through the cancer care continuum. Coordinated interdisciplinary care and connected patients to financial, lodging, mental health, and community resources while assisting with treatment-related documentation.

Coordinated referrals by reviewing referral details, requesting additional records, and obtaining insurance authorizations. Scheduled new and follow-up patient visits with interventional and orthopedic spine providers and supported referral status updates with referring offices.

Coordinated care with providers, primary care teams, and social services agencies to support members through education and resource navigation. Conducted telephonic assessments and planned, monitored, and evaluated care options to improve health outcomes and coordinate services.

Monitored staff adherence to agency procedures and ensured completion of required documentation to maintain quality services. Tracked medication incidents, reviewed incident reports with supervisors, conducted site visits, and monitored clients’ physical environment and health needs for safety concerns.

Collaborated with RN case managers and clinical social workers to identify high-risk members and support disease management and care plan adherence. Assisted with discharge planning in skilled nursing facilities and helped close preventive care gaps while coordinating with providers, staff, and family members.

Provided resource referrals for members with social determinants of health needs and helped individuals navigate the health system in the Portland Tri-County and Clatsop County areas. Identified members needing coordination early by monitoring emergency department use and inpatient admissions/readmissions and supported care through team-based coordination.

Earned a Bachelor of Arts in Social & Behavioral Studies from George Fox University, completed in December 2008.
