Gina Rago

I am a Licensed Clinical Social Worker with 20+ years of experience advancing population health outcomes for medically and socially vulnerable adults across Medicare, Medi-Cal, D-SNP, and CalAIM. Throughout a 23-year Kaiser Permanente career, I’ve been recognized as a trusted clinical consultant and program builder across public and payer-based healthcare systems at scale.

I lead population-based integrated care management for approximately 3,000 Medicare and Medi-Cal D-SNP dual-eligible members, navigating benefit structures to remove access barriers and connect members to medically necessary services and community resources. I complete thousands of biopsychosocial assessments and coordinate across primary care, specialty care, behavioral health, hospitals, skilled nursing facilities, and community partners to improve continuity and outcomes at scale. In quality and utilization work, I support HEDIS and CMS Stars activities within Kaiser’s Complex Chronic Conditions program, managing a caseload of approximately 40 high-acuity patients and partnering on corrective action plans and regulatory documentation.

I also build and scale clinician and caregiver education programs, including founding “Caring for Loved Ones with Memory Loss,” delivered to an estimated 1,600–2,100 caregivers through 100+ classes. As a Regional Motivational Interviewing Trainer, I deliver 60–90 multidisciplinary trainings across Northern California, and I create standardized community resource packets that strengthen access and reduce preventable hospitalizations.
