I’m a care coordination leader with 7+ years supporting Medicare and medically complex patients across post-acute, skilled nursing, and home-based settings. I focus on managing transitions of care, removing barriers tied to access and social needs, and strengthening follow-through.

In my current role as a Remote Sr. Clinical Administrative Coordinator, I launched the care coordination function as the first coordinator in the market. I built and standardized workflows that scaled the program into a multi-state operation, improving consult tracking, documentation, and escalation of patient needs.

I oversee care coordination for 450+ Medicare members across skilled nursing and post-acute settings. I coordinate services including SNF admissions, therapy (Part A & B), specialty consults, and DME—while tracking Medicare STAR Measures and escalating discrepancies to maintain continuity, compliance, and quality.

I’m known for strengthening provider collaboration and supervising Clinical Care Coordinators across four markets with guidance on patient needs, workflow priorities, and documentation standards. I developed a referral tracking tool that improved visibility across the team and reduced missed or delayed services, and I support onboarding and training as programs expand.
