Dayna Machuca

At Masada Community Mental Health Services, I coordinate healthcare and community-service referrals for Medi-Cal members and families, helping connect them with providers, transportation, specialty resources, and other supports that sustain continuity of care.

I conduct structured outreach and follow-up across phone, email, letters, and in-person contact. I work with L.A. Care, Anthem, Health Net, providers, clinical staff, and care-management teams to clarify needs, resolve barriers, and document referral activity in CareLogic and electronic records.

Previously, at HOLA HHSS, I managed a caseload of approximately 40–45 clients while coordinating appointments, benefits, resources, and time-sensitive follow-up across multiple agencies.

My medical assisting background at HOLA Recuperative Care and a family-practice externship gives me hands-on experience with patient intake, vital signs, scheduling, referrals, medical terminology, clinical documentation, and HIPAA-compliant care coordination.
