Conduct telephonic outreach and home visits for postpartum Medicaid members, assessing healthcare and social service needs. Develop individualized care plans, coordinate postpartum care and community resources, and maintain HIPAA-compliant member documentation.

Respond to member/provider inquiries by phone and written correspondence within established timeframes. Coordinate transportation and referrals, investigate claim payment inaccuracies, and manage patient billing by entering information into a CRM and supporting pharmacy order distribution.
