Primary ResponsibilitiesPerforms comprehensive evaluations and documents findings in a concise/comprehensive manner that is compliant with documentation standards for the Center for Medicare and Medicaid Services (CMS)Performs patient assessments telephonically, virtually, or in the patient's home as neededUnderstands and plans to assure services provided work within the boundaries of the patient's plan eligibility. Coordinates identified patient's needs utilizing federal, state, and local community resources, as availableCoordinates with external and internal teams to minimize obstacles and increase in patient and provider satisfactionActively participates in staff and Interdisciplinary Team (IDT) meetingsAdheres to organizational and departmental policies and proceduresMaintains a high level of professionalism and adheres to the WellMed Core ValuesAssumes responsibility for personal and professional developmentEnsures licensure, certifications, and annual training are maintained and compliantMaintains knowledge of current health plan benefitsConducts advanced care discussions with a patients and their families and properly documents their wishes in the electronic medical recordExhibits professionalism and is courteous with all patients, physicians and co‑workersFollows CareCoach Connect providers' orders regarding the scope and frequency of services needed based on acuity and patient/family needsMaintains a patient case load, daily visits and point of care documentation levels as per standardsDocuments in the electronic health record progress toward established goals as per standardsPerforms other duties as assignedRequired QualificationsBachelor of Social Work with a minimum of 3 years experience beyond the required years of experienceCurrent BLS certification or ability to obtain within 30 days of employment hire date3+ years social work experience in a health care environmentProficient in MS Office Suite to include Word, Excel, Notes, Outlook, and other departmental specific programs/applicationsAbility to prioritize, plan, and handle multiple tasks/demands simultaneouslyMust reside within the state of TexasPreferred QualificationsMasters of Social Work with licensure (LMSW or LCSW or LBSW)Experience working with geriatric and behavioral and mental health patient populationsLong Term Care experienceKnowledge of disease management, managed care, medical terminology, referral process, claims, and CPT codingBilingual (English/Spanish) language proficiencyExcellent oral and written communication skillsSolid organizational skillsPhysical & Mental RequirementsAbility to lift up to 50 poundsAbility to push or pull heavy objects using up to 100 pounds of forceAbility to stand for extended periods of timeAbility to use fine motor skills to operate equipment and/or machineryAbility to receive and comprehend instructions verbally and/or in writingAbility to use logical reasoning for simple and complex problem solvingOccasionally requires exposure to communicable diseases or bodily fluidsAll employees working remotely will be required to adhere to UnitedHealth Group's Telecommuter Policy.Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $49,700 to $88,800 annually based on full-time employment.
