United States onlyOne Medical is seeking a Virtual RN - Impact Program nurse to provide advanced clinical care to patients with cardiometabolic conditions. The nurse will work as part of a team to manage patient care, provide education and counseling, and triage patients. The position is full-time and virtual.
Requirements
- Active RN license with NCLEX, active California, New York and/or compact multi-state RN licensure required
- Bachelor of Science in Nursing preferred or Associate of Science in Nursing
- 2+ years of nursing experience in Primary Care or working with patients within the scope of diabetes and chronic disease management
- Certification in Diabetes Education or expectation to achieve this within the first year, CDCES certification
- Excellent clinical and communication skills
- Strong interest in using technology to deliver high quality, evidence-based primary care
