What I'm looking for

I’m looking to leverage my RN background as a Care Manager or Telephonic Nurse, delivering assessments, care coordination, and medication education for complex chronic patients—supporting adherence, reducing readmissions, and improving health outcomes in remote care.

I’m a bilingual RN (English & Spanish) with 7+ years of clinical experience across home health, progressive care, med-surg, and skilled nursing. I provide telephonic/remote care through patient assessments, telephonic follow-ups, medication education, and patient/caregiver education for individuals with complex and chronic health conditions.

In my current role, I manage care for patients with chronic and complex conditions using in-home and telephonic interactions, developing individualized care plans with physicians, pharmacists, and interdisciplinary teams.
