I’m a compassionate Health & Wellness care professional focused on empowering people to build sustainable healthy lifestyle habits through education, accountability, and evidence-based coaching. I bring experience in chronic care management (CCM), patient education, care coordination, and healthcare informatics, with a strong commitment to patient engagement and measurable wellness goals.

In my current Care Professional / Care Coordinator role (Remote), I manage a caseload of 140+ CCM patients. I deliver personalized wellness coaching to improve medication adherence, preventive care, and chronic disease self-management, while maintaining trusting relationships through monthly outreach and education.

I coordinate care across physicians, specialists, caregivers, and patients, ensuring continuity and clarity across the care journey. I maintain care plans and documentation in CrossTx and EHR systems, and I ensure HIPAA-compliant communication and documentation to support safe, reliable care.

Earlier, I supported patient services and operations as an Administrative Assistant & Care Coordinator, coordinating scheduling, intake, authorizations, and documentation. I also contributed clinical support and office support through consulting work in private clinics and research/clinical assistant experience, and I’m currently advancing my training with an M.S. in Healthcare Informatics (Expected Oct 2026).
