At Independent Living Systems, LLC, I provide housing navigation and care coordination within CalAIM Community Supports. I use eCare and integrated Epic portals to manage client data and compliance documentation across Kaiser Permanente, HealthNet, and Anthem systems.
At Titanium Healthcare, I provided housing navigation and case management under CalAIM initiatives, assisting members with referrals, complex documentation, and cross-system care coordination.
At Peach Tree Healthcare, I delivered PrEP navigation and sexual health education, and maintained patient charts and compliance documentation in NextGen. At WellSpace Health, I provided HIV/STI prevention education and medical support and used OCHIN Epic for patient charting, referrals, and integrated case management.
At Harm Reduction Services, I delivered Ryan White–funded case management for clients living with HIV/AIDS and facilitated access to services including Medi-Cal, CalFresh, SSI, housing, and transportation. I also contribute to HIV/AIDS care planning as a member of the HIV Health Services Planning Council (Sacramento County).
