At Humana SNP/Sagility Health, I develop individualized care plans using the 5Ms framework, complete post-hospital and ED follow-ups, reconcile medications, address social determinants of health, and resolve member issues involving benefits, claims, eligibility, and pharmacy needs. I maintained 99% documentation accuracy for complex member care plans while using CRM and care-management systems to support CMS and SNP compliance.

At Novocure, I guided patients and providers in using the Optune Device, coordinated telephonic outreach and transitions of care, and connected patients with housing, food, social work, and mental health resources. At Houston Methodist Hospital Neurosurgery, I coordinated patient flow, documented care in EMRs, managed authorizations, supported case management, and served as a clinical resource and preceptor.
