What I'm looking for

I’m looking for a role where I can support mental health services with strong client-centered coordination, meaningful research-backed insights, and team development—continuing to build continuity of care while growing into direct-practice social work.

I’m a mental health and care coordination professional with a background in psychology and health communication, specializing in client intake, referral coordination, and continuity of care. Over the past three years, I’ve worked in both private practice and telehealth settings, supporting individuals navigating mental health challenges, complex needs, and barriers to accessing care.

At Myers-Galloway Counseling, I served as the sole Client Success Coordinator for a group private practice, where I was responsible for managing the full client journey from initial inquiry through ongoing care. I conducted intake screenings, scheduled appointments, verified client needs, and coordinated referrals across behavioral health, psychiatric, and community-based services. I also built and maintained a referral network by conducting outreach with local providers to establish strong care partnerships, ensuring clients were connected to culturally responsive and appropriate services. My role required managing high-volume communication, maintaining accurate documentation, and creating a welcoming, supportive experience for every client.

Previously, at PlushCare, I worked as a Care Coordinator in a fast-paced telehealth environment, managing a high volume of daily patient interactions. I supported appointment scheduling, prescription coordination, lab follow-ups, and specialist referrals while maintaining EHR accuracy and adhering to strict quality and performance metrics. I also assisted in training new hires and responding to time-sensitive and high-priority patient needs, including those experiencing mental health crises.

Across my roles, I’ve developed strong skills in patient communication, administrative coordination, and navigating healthcare systems with both efficiency and empathy. I’m particularly interested in positions such as intake receptionist, client coordinator, referral specialist, and discharge planner roles within mental health agencies and hospitals, where I can continue supporting access to care, improving patient experience, and ensuring individuals receive the services they need to move forward.
