What I'm looking for

I’m looking for a social work role where I can coordinate care, assess Health-Related Social Needs, and advocate for clients—working closely with interdisciplinary teams to improve service access and follow-up.

I’m a Master’s-level Social Worker with experience in care coordination, behavioral health, social needs assessment, community resource navigation, and client advocacy. I’ve supported Medicaid populations by conducting assessments and coordinating services, including counseling support to diverse populations.

In my most recent role as a Social Care Network Specialist, I served as the primary contact for Medicaid members, uncovered Health-Related Social Needs (HRSN) using Unite Us, and helped members access services for food security, housing stability, and transportation access. I value documentation, client engagement, and interdisciplinary collaboration—using regular reporting and case conferencing to track progress and improve service delivery.
