What I'm looking for

I seek a clinical case management role using my CCM and 20+ years of nursing experience to improve patient outcomes, support interdisciplinary teams, and contribute to quality improvement in a collaborative healthcare setting.

I am a bilingual (Spanish) Registered Nurse with over 20 years of clinical experience across pediatric, adult, maternity, and transplant care settings. Over the past eight years I have focused on complex case management and utilization review, achieving my CCM in 2021 and PALS in 2024.

I have managed high-risk and complex members, coordinated multidisciplinary teams, and acted as a liaison between patients, community resources, and medical staff to improve outcomes and access to care. I have experience with Epic, Cerner, JIVA, SharePoint, and other EMR platforms.

My background includes inpatient and outpatient roles at large healthcare systems and specialty hospitals, work with worker's compensation cases, and development of process guides and training for new case managers. I routinely conduct utilization review, discharge coordination, authorization management, and patient education to reduce readmissions and optimize care plans.
